Privacy Practices

Please bring all the following to your appointment:

• MRI, CT Scan or X-ray Discs/ Films as well as the reports related to your condition
• Any EMG/NCS Reports done in the past
• Completed Patient Registration Packet which you are able to print above
• Name and numbers of referring and other treating physicians
• Medical records such as OP Reports of any injections, surgeries or procedures pertaining to your condition done in the past
• List of current medications and health conditions
• Current Photo ID and Insurance Cards
